Thought Diary 3
Overview
This CBT handout provides a structured template for recording and analyzing distressing thoughts through a five-step process: identifying activating events, beliefs, consequences, engaging in detective work to challenge those beliefs, and formulating balanced alternative thoughts. It is designed for clients to self-monitor emotional responses and cognitive distortions by rating the intensity of feelings and belief strength on a scale of 0 to 100. The resource includes prompts for evaluating factual evidence and considering alternative perspectives to replace unhelpful thinking styles.
