Thought Diary 2
Overview
This CBT handout provides a structured template for recording activating events, associated beliefs, and emotional consequences, including ratings of belief and distress intensity. It guides users to identify unhelpful thinking styles and engage in 'detective work' by examining factual evidence for and against their most distressing thoughts. The resource is designed for clients to practice cognitive restructuring techniques independently.
